About SINE Workflow System

Greene County General Hospital has implemented a COVID-19 Vaccine Mandate policy in accordance with the Department of Health and Human Services and Centers for Medicare and Medicaid Service Code 4120-01-P. This requires that any contractors, service providers, or individuals (other than patients or visitors) that regularly enter a GCGH facility or directly interacts with GCGH employees provide proof of COVID-19 Vaccination or proof of exemption from a COVID-19 vaccine mandate from their employer. The policy also requires that all students participating in learning onsite in a GCGH facility must be fully vaccinated for COVID-19. Individuals required to comply with this policy must submit this documentation to be permitted entrance to any GCGH facility as a vendor, contractor, or student. Individuals must also check-in to the visitor/vendor management system each time they enter a GCGH facility and complete a COVID-19 Screening Questionnaire.

Instructions for submitting your documentation:

  1. Click the link for your role. You will be taken to the Sine Logon Page. (Sine is the software system utilized by GCGH to confidentially track and monitor compliance with the COVID-19 Vendor/Student Vaccine Mandate.)

  2. Click “Sign up” if you do not have a Sine logon.

  3. Click on “Free access” under the “User account” option.

  4. Follow the prompts to enter your individual information.

  5. Once your account is created, you will be taken to the form to submit your documentation.

  6. System will prompt you to enter your employer or school name.

  7. System will prompt you to select if you are vaccinated or have an approved exemption.

  8. Click Next.

  9. Click “browse files” and select the file for your proof of vaccination or proof of exemption. (Proof of exemption must be on employer letterhead.)

  10. Read the COVID-19 policy agreement statements.

  11. Sign your name.

  12. Click Next

  13. Click Submit

You will receive email confirmation that your documentation has been submitted. You will receive an additional email when your submission is approved. Be sure to use the same name and email address to check-in when you arrive at a GCGH facility.

You can expedite your check-in by downloading the Sine App on your mobile device. You can then use your mobile device to scan the QR code at the check-in kiosk. You will complete the COVID-19 Screening questions on your mobile device and your badge will print at the check-in kiosk.
