CDL Physical information
Certain providers at our My Bloomfield Clinic are certified to perform CDL physicals. If you would like to schedule a physical, please call our My Clinics 812-847-4481 and we will schedule you with a certified provider. Please bring your completed Medical Examination Report Form (MCSA-5875) and Medical Examiner’s Certificate Form (MCSA-5876) with you to your appointment. CDL physicals cost $100; due at the time of your appointment.

Sarah Flemming, M.D.

Sarah Flemming, M.D. is a board-certified Pediatric Doctor of Medicine (M.D.) providing care at My Bloomfield Clinic. Dr. Flemming graduated from Indiana University School of Medicine and completed her pediatric residency at the University of Utah. Dr. Flemming is certified by the American Board of Pediatrics. Pediatricians treat children, infants, and adolescents. They are trained to meet the unique needs of children, through all of their developmental stages, as they grow and mature. Dr. Flemming grew up in Bloomfield and enjoys providing care to the residents of Greene County. She currently lives in Bloomington with her family.

Ariel Feltner, FNP-C

Ariel Feltner, FNP-C, is a Family Nurse Practitioner who has served our communities for years. As a graduate of Bloomfield High School, she has excelled throughout her career. After graduating with honors from Indiana State University with a Bachelor of Science in nursing, she went on to graduate with a Master of Science and Nurse Practitioner License from Ball State University. Ariel chose to join the My Bloomfield Clinic because she is dedicated to providing trusted and professional care to our community. Ariel lives in Bloomfield and enjoys spending time outdoors with her family.

Theodore Hoppe, D.O. - Internal Medicine & Pediatrics

Dr. Theodore Hoppe is an experienced Internal Medicine & Pediatric physician providing comprehensive care to adults and children at both My Linton and My Bloomfield Clinics. Dr. Hoppe graduated from Edward Via Virginia College of Osteopathic Medicine in 2008. He spent an additional year of student fellowship studying osteopathic manipulation. Dr. Hoppe also graduated from SUNY Buffalo State with Medical Education Training: Internal Medicine-Pediatrics in 2012. He served as the Pediatric Medicine chair for the Fairview Health Services, as well as an Osteopathy, Internal Medicine, and Pediatrics physician for Fairview Health Services. Dr. Hoppe is passionate about improving the quality of life for people with multiple health issues and supporting parents and children with pediatric illnesses. Dr. Hoppe also enjoys caring for newborn babies and provides care at Greene County General Hospital. He and his family recently relocated from Minnesota. He now resides in Linton, Indiana and enjoys spending his free time outdoors.

Renee Rottet-Aufderheide, MSW, LCSW

My Clinics offer a clinical social worker, Renee Rotett-Aufderheide, MSW, LCSW, at all four locations. Graduating with honors from Indiana University, Renee has more than 26 years of diverse experiences, especially in Clinical Social Work. As a Clinical Social Worker, Renee provides direct services, including interventions focused on interpersonal interactions, intrapsychic dynamics, and life management issues.
